Ratings of the Nagaland Health Project for India were as follows: outcome was satisfactory; Bank performance was highly satisfactory; M&E quality was high. Lessons learned and recommendations include the following: A small pilot conducted early in implementation is invaluable for testing and refining systems and procedures, but it should include structured feedback from implementers and beneficiaries and allow flexibility to adjust design based on what works. Developing complex information technology systems benefits from a phased approach guided by a clear master plan that ensures interoperability; start with applications that have the strongest and clearest use cases, and build in iterative cycles of development, testing, and adjustment with realistic timelines. Providing facilities and committees with discretionary funds linked to results through results based financing (RBF) empowers local decision-making, strengthens a culture focused on outcomes, and builds planning, budgeting, and accountability skills; to enhance impact, align RBF indicators with national targets and existing data systems to reduce reporting burdens, and consider allowing more local discretion in indicator selection to better match population needs, while using clear, low-cost independent verification methods and recognizing the administrative demands RBF entails. Longer-term engagement through repeat operations is recommended to consolidate gains, sustain capacity, and build on the relationships and systems established, thereby deepening quality improvements and extending successful approaches.
